I use this plugin and is satisfied with its work.
I have a standard theme, with it the plugin works fine, there have not been any problems yet. The client liked the look of the form.

This piece of junk broke my entire site. Currently restoring to a backup. Uninstalling the plugin does not resolve the issue. DO NOT USE THIS PLUGIN IT WILL BREAK YOUR WEBSITE.
